We complete a log book and significant events daily. If we feel that there is something that is significant to report we will contact the social worker or their team. If they are unavailable or it is out of hours we will contact the Emergency Duty Team. We will also inform any other relevant agencies such as the Police.
- We work closely with all professionals and support the young person with all appointments
- We endeavor to support all young people to engage in education or employment.
- We will taylor a “Pathway to independence” plan based on the young persons needs in order to achieve all outcomes
- We will provide weekly updates on the young person
- We will report monthly via email a review of the young persons current outcomes
- Our staff will work closely with the social worker so that the young persons needs are met according to their current pathway plan.
- We will attend all reviews and provide our own review of the young person based on their progress in placement
- We will respond to all calls and emails within at least 24 hours
- Each young person will be assigned a keyworker that will support them
